Like in the original film, Alfred is sick with MacGregor's Syndrome except it's built up and developed better. Because of this, Batman becomes secretly worried of having to lose his family again, causing a rift between the Batfam. We still keep that conversation between Bruce and a sick Alfred because that was one of the scenes in the original I remember thinking worked well. Alfred gets cured in the end thanks to Freeze already finding a cure for his stage of MGS.
