Set in Sicily from 1989 to 1991, the film follows a brutal succession war between the Corleonesi and the Ndrangheta as political assassinations, retaliatory massacres, and shifting allegiances turn a family empire into a collapsing kingdom. At the center is Toto Riina, whose command of violence masks a deeper fear of being replaced, outmaneuvered, or rendered irrelevant by the very machine he built.
The story moves from a shocking opening assassination into a prolonged war of retaliation, then into uneasy peace, betrayal, state pressure, and public backlash after the murders of anti-mafia prosecutors and judges. By the end, the men who thought they were controlling history are trapped by it.
